Terms & Conditions
01Medical Service Scope & Clinical Disclaimer
All medical services at Everyday Wellbeing Guide are provided by licensed, board-certified healthcare practitioners. Information on our platforms is educational and does not replace professional medical advice, diagnosis, or treatment. Always consult a qualified clinician regarding any health condition or medication adjustment.
02Appointment Scheduling & Cancellation Policy
Medical appointments must be scheduled in advance. Cancellations require a minimum 12-hour notice to allow reallocation to other patients in need. Repeated no-shows may result in a scheduling hold. Urgent care visits are accommodated based on clinical priority and availability.
03Informed Treatment Consent & Procedures
Prior to any medical procedure, your treating provider will explain the diagnosis, proposed treatment, expected outcomes, potential risks, and available alternatives. Written informed consent is required for all procedures involving sedation, surgery, injections, or aesthetic interventions before treatment commences.
04Telehealth Service Limitations & Terms
Virtual consultations have inherent clinical limitations compared to in-person visits, including the inability to perform physical examinations or diagnostic testing. Telehealth is not suitable for emergencies. Prescriptions issued via telehealth follow regulatory guidelines and may require in-person follow-up for controlled medications.
05Patient Health Records & Access Rights
Patients have the right to access their complete medical records, lab results, and treatment history through the patient portal. Requests for record transfers to other healthcare providers must be submitted in writing. A reasonable administrative fee may apply for extensive record compilation or duplicate copies.
06Patient Code of Conduct & Care Responsibilities
All patients are expected to treat clinical staff, fellow patients, and facilities with respect. Disruptive, abusive, or non-compliant behaviour that compromises the safety or wellbeing of others may result in discharge from the practice. Patients are responsible for providing accurate medical history and adhering to agreed treatment plans.
07Financial Responsibilities & Billing
Patients are responsible for all charges associated with their care. Co-payments and deductibles are due at the time of service. While we assist in filing insurance claims, the patient remains financially liable for any outstanding balances, uncovered services, or denied claims according to their individual insurance policy.
08Prescription Management & Refill Requests
Prescription refills should be requested through the secure patient portal or directly with your pharmacy at least 48 hours in advance. Providers generally will not authorize routine refills after hours or during weekends. Changes to medication regimens require a scheduled clinical evaluation.
09Insurance Authorizations & Third-Party Payer
Our administrative team makes reasonable efforts to obtain necessary prior authorizations for specific treatments or imaging. However, we do not guarantee insurance approval. It is the patient's responsibility to verify their specific network coverage, benefit limits, and active status prior to receiving care.
10Electronic Communication & Secure Messaging
The secure patient portal is the designated platform for discussing clinical questions and sharing health documents. Standard email and SMS text messaging are not entirely secure methods for sensitive health data and will only be used for general administrative notices, such as appointment reminders.
11Modifications to Services & Clinic Policies
We reserve the right to modify operating hours, service offerings, clinic locations, and these Terms and Conditions at any time to ensure the continuous delivery of high-quality care. Any material changes to our administrative policies will be communicated to active patients via the portal or posted within our facilities.
12Platform Availability & Limitation of Liability
While we strive to maintain uninterrupted access to our digital health platforms and patient portal, occasional downtime for scheduled maintenance or unforeseen technical issues may occur. The practice shall not be held liable for indirect or incidental damages arising from the temporary unavailability of these electronic services.
